Jake’s Village Pizzeria, 4 State St.

• CRITICAL VIOLATIONS: Slow-cooker of meatballs and sauce 110 degrees, owner said meatballs had been microwaved an hour before and placed in slow-cooker; instructed to microwave meatballs to 165 and hot hold back in slow-cooker at 140 degrees.

• NON-CRITICAL VIOLATIONS: No screen on open front door; back door screens have gaps at top and bottom allowing insect and rodent entrance; light in walk-in freezer broken; light shields missing from fixtures in kitchen above coolers.
