• NON-CRITICAL VIOLATIONS: Rear door to exterior has gaps, not weather tight, repeat; floor tiles missing in areas on west side of building, repeat; wall above rear prep counter is water damaged, plaster has bubbled out, repeat; several water damaged and missing ceiling tiles throughout the facility, repeat.

• CRITICAL VIOLATIONS: Sliced green beans on shelf in kitchen temped at 48 degrees, had been removed from cooler less than an hour before, operator voluntarily placed beans in cooler.

• NON-CRITICAL VIOLATIONS: Water-damaged ceiling tiles in kitchen, interview indicates leak was just fixed; some food items in cooler wrapped in newspaper; uncovered bag of rice and flour in storage room; container of sliced beef on kitchen floor; container of sliced beef thawing temped at 32 degrees, operator voluntarily moved bin to sink under running water; rice scoop in rice cooker lacks handle, scoop with handle procured.

• CRITICAL VIOLATIONS: Cold hold in center island out of temperature, between 50-53 degrees, island unit emptied every night and all potentially hazardous foods had been in unit about 1 hour as per manager, all foods removed to walk-in cooler, corrected.

• NON-CRITICAL VIOLATIONS: Chlorine in sanitizing solution too high, reviewed proper dilution technique, corrected.
