Big Nick Cafora’s, 105 Telegraph Road

• CRITICAL VIOLATIONS: Owner used bare hands to place celery on plate of chicken wings, celery discarded and gloves put on; two-door cooler near ice machine had ambiant temperature of 55 degrees, shredded mozzarella cheese in cooler was 62 degrees, owner said cheese taken out of cooler, shredded and placed back in cooler 1 1/2 hours before, cheese moved back to walk-in so cooler can be repaired.

• NON-CRITICAL VIOLATIONS: Shelving in walk-in cooler has peeling paint and grime accumulation; grease and grime buildup on equipment throughout kitchen; hand-wash sink in pizza area dirty; soap dispenser in men’s restroom empty; back screen door has gap at bottom of door and bottom of screen not secured, repeat; floor under and around equipment in kitchen and pizza area has debris accumulation; fan covers in walk-in cooler have debris buildup; light shields missing from fixtures by mixer and three-bay sink.

• CRITICAL VIOLATIONS: Beef and chicken bases out of temp, cook reported they had been made within 30 minutes but had not been temped before placing in hot hold, both were reheated to correct temperature before return to hot hold.

• NON-CRITICAL VIOLATIONS: None.

Casey’s Malt Shoppe, 3041 Military Road

• CRITICAL VIOLATIONS: Raw eggs found in a bin in top of prep table cooler with ambiant temperature of 49 degrees with open cover, other foods in bins of top cooler were 45 degrees or lower, cook reported eggs put out short time before, eggs voluntarily moved to under-counter cooler to continue cool storage and top was covered.

• NON-CRITICAL VIOLATIONS: Ice scoop found in contact with dusty surface on top of ice machine, staff voluntarily washed, rinsed and sanitized scoop, which was properly stored; accurate thermometer missing from under-counter cooler, corrected.

• CRITICAL VIOLATIONS: None.

• NON-CRITICAL VIOLATIONS: Entrance to walk-in cooler worn, and not smooth and easily cleanable.

• CRITICAL VIOLATIONS: Vegetable rolls out on countertop at 77 degrees, cook said rolls were partially cooked and placed on counter to further cook as needed, rolls voluntarily discarded.

• NON-CRITICAL VIOLATIONS: Plasticware stored with handles facing down; no sanitizing solution available, repeat, corrected; horizontal table and shelf surfaces not clean; no paper towels in dispenser in restroom; styrofoam takeout containers stored face up.

• NON-CRITICAL VIOLATIONS: Thermometer missing from refrigerator in dish room; hot water handle on right side of three-bay sink in dish room is broken.

• NON-CRITICAL VIOLATIONS: Food uncovered in walk-in cooler; thermometer in cooler only reads down to 100 degrees; terry-cloth towels lining the bottom of the flip-top cooler on the opposite side of the cook line, not smooth and easily cleanable; handles not clean on True refrigerator.

• CRITICAL VIOLATIONS: One can of food product found in canned storage severely dented, repeat, cook removed to office for return to supplier; food-testing thermometer not properly calibrated, re-calibrated.

• NON-CRITICAL VIOLATIONS: Food product stored on floor, on milk crates and on low shelving, which does not allow for easy access to clean beneath; in-use serving spoon on cook line found in pan of standing water between uses; utensils stored in cup with food contact surfaces out, repeat; bare wood surfaces on shelving in basement, cardboard lining shelves in kitchen, neither smooth and easily cleanable, repeat; gaskets of most cooler drawer doors are torn; bottom of freezer has spillage, most gaskets of cooler doors have debris buildup and shelving in most coolers have buildup; men’s room door is not self-closing.
