• CRITICAL VIOLATIONS: About 2 pounds of uncooked chicken wings were 50 degrees and cooked taco meet was 50 degrees in walk-in cooler, both were placed there an hour before, thermostat to be adjusted to below 45 degrees; exterior walk-in cooler thermometer is 55 degrees.

• NON-CRITICAL VIOLATIONS: Rodent droppings observed along west basement wall, dead mouse observed in trap in same area.

• CRITICAL VIOLATIONS: None.

• NON-CRITICAL VIOLATIONS: Rear window open, lacks screen; bar area in floor in disrepair, missing tiles; cup without handle used as sliced pepper scoop in line cooler, cooks use gloves when handling, proper scoop with handle located.

• NON-CRITICAL VIOLATIONS: Rear door to exterior has gaps, not weather-tight, repeat; floor tiles missing in areas on west side of building, repeat; wall above rear prep counter is water-damaged, plaster has bubbled out, repeat; several water-damaged and missing ceiling tiles throughout facility, repeat.

• CRITICAL VIOLATIONS: Drain line above basement prep room doorway leaks onto basement floor; two oysters in Pepsi cooler lack tags, voluntarily discarded; two ray oysters stored on shelf above sliced cold cuts and bread in Pepsi cooler, no apparent contamination, voluntarily discarded; uncooked fish filet on shelf above containers of tartar sauce in two-door cooler, fish moved below ready-to-eat food.

• NON-CRITICAL VIOLATIONS: Slicer in basement soiled with dried food particles; kitchen in need of general cleaning, repeat; pizza cutter, spatulas stored between gaps of wire shelf, which is soiled; fresh water line has leak in basement, two buckets below catching water, buckets of standing water have become rancid; drain line above doorway to basement has leak; no paper towels at kitchen hand-wash sink, repeat, corrected; soap dispenser at kitchen hand-wash sink is broken, no other soap dispenser in area, repeat; men’s room hand dryer out of order, no other hand drying available; exterior of grease bin is heavily soiled; several uncovered buckets at grease bin area, repeat; standing water on basement floor at grease trap, repeat; basement prep room and walk-in cooler floors soiled with old food debris, repeat; debris, tall grass and weeds around perimeter of building.
