• CRITICAL VIOLATIONS: Potentially hazardous foods not kept at or below 45 degrees during cold holding, all temperature-controlled for safety foods were voluntarily discarded, cooler to be looked at and adjusted; not enough refrigerated storage equipment present, properly designed, maintained or operated so that all potentially hazardous foods are cooled properly and stored below 45 degrees as required.

• NON-CRITICAL VIOLATIONS: High-temperature dishwasher not sanitizing; paper towel dispenser at hand-wash station empty; light shield missing from fixtures above mixer, near alcohol coolers, light shield cracked above dishwasher.

• NON-CRITICAL VIOLATIONS: Sanitizer not available for wiping cloths.

• CRITICAL VIOLATIONS: Inspector tested sanitizer bucket to be improper dilution, correction.

• NON-CRITICAL VIOLATIONS: Front and back door open allowing for possible insect or rodent entrance, both doors closed and corrected; ceiling in walk-in has debris, floors under shelving in walk-in have debris; lights in basement are not shielded.
