• CRITICAL VIOLATIONS: Freezers in rear shed heavily fire-damaged, items still in freezer at time of inspection, told not salvage any items, repeat April 23 and April 24; bar cooler out of order due to tripped circuit due to fire, all sliced fruits and juices found to be below 45 degrees, all items moved to walk-in cooler.

Pete’s Lebanese Bakery, 2468 Elmwood Ave.

• CRITICAL VIOLATIONS: Roast chicken in hot hold at 95 degrees, was removed an hour before, employee rapidly reheated chicken to 165 degrees in boiling water, to be held at 140 degrees in steam table; roast duck in hot hold unit at 58 degrees, was removed from cooler and placed in unit 30 minutes before, employee voluntarily placed duck back in cooler; hot hold box not holding food at 140 degrees or above, all food removed from unit.

• NON-CRITICAL VIOLATIONS: Front counter hot box lacks thermometer, repeat, corrected as of April 12 inspection.

• CRITICAL VIOLATIONS: Spray bottle of degreaser stored hanging from shelf above prep counter in rear building, no apparent contamination, employee moved spray bottle away from food contact surfaces; two raw eggs in carton stored above sliced vegetables in cooler in rear building, no apparent contamination, eggs moved away from ready-to-eat food.

• NON-CRITICAL VIOLATIONS: Slice has food remnants stuck on it, not cleaned and sanitized following last use from night before; ice scoop bin in rear building soiled; gaps observed at bottom of double doors and garage door in front building; openings, gaps observed in foundation wall around rear building; door from parking lot to rear building has gaps at bottom; mouse droppings observed on floor in rear right corner of rear storage room in rear building; wall at slop sink and baseboard next to walk-in cooler in rear building is deteriorated; cobwebs observed in corners where wall meets ceiling, on duct work and lights in dining room; wall below dishwasher in front building is deteriorated, wall board crumbling.
